Long term care definition – A collection of personal and health services dedicated to enabling people to live safely and as independently as possible when a gradual or sudden illness, injury or other condition doesn’t allow them to perform everyday tasks on their own. Long term care can last months or years and is based on each individual’s abilities and condition. Care can include from family and friends or paid professionals and may be provided in their home or at day programs, assisted living facilities or nursing homes.
